(Q) I am caring for my mother, but something has come up and now I need someone to care of her for a short period of time. Can Sylvan Crossings help? (A) Sylvan Crossings offer short-term stay programs, also called Respite Care that allow home caregivers to take vacations or attend to needs at home or work. Your loved one will receive quality care in a comfortable and pleasant home environment, surrounded by many opportunities to participate in leisure programs and make new friends.
(Q) I am worried about my father. He lives at home alone. I notice his appearance has deteriorated and he is not eating very well. Can Sylvan Crossings help? (A) Yes we can help. Sylvan Crossings team members provide assistance and encouragement with personal cares, including bathing, grooming, and dressing. We offer opportunities to socialize with others living in our communities. We have leisure areas for individuals to spend with families, friends and others who live in our homes. We encourage individuals to become involved in our activity programs. We complete a leisure assessment to assist team members in coordinating programs that are meaningful to the individuals residing in a Sylvan Crossings home. Three home-cooked meals are served daily along with snacks being available. A nutritional assessment is completed periodically to identify individuals current food likes and dislikes. Team members provide encouragement and reminders to maintain good health through good nutrition.
(Q) How will Sylvan Crossings monitor my mother’s health status? (A) Sylvan Crossings has policies and procedures that assist us in determining each individual’s health status on an ongoing basis working with each individual’s physician. Sylvan Crossings completes and reviews:
- Case management reports
- Assessments for cognitive status, functional status, risk factors,
- Nursing, nutrition, leisure and quality of life status.
- Practitioner medical plan of care
- Medication reviews
- Daily monitoring and documentation
- An Individualized Service Plan that details the services Sylvan Crossings will provide
Practitioners are made aware of their patients status when it has changed and as frequently as the changes occur. It is our policy to keep continuity with each Practitioner regarding their patients medical status. Sylvan Crossings encourages continuity in relationships with all healthcare providers and will assist in making appointments or in assisting in contacting new health care providers. |
