| | Levels of Care
Three levels of Care have been established at Sylvan Crossings. Each is progressive in the scope and intensity of the amount of care required. The monthly fee increases for each level of care. Each level builds upon the previous level.
Basic Care
Examples: Weekly bathing assistance, daily personal grooming, activities, medication ordering and dispensing, toileting assistance, ambulation help, meals and snacks, laundry and housekeeping services. Basic care is included in each monthly suite rate.
Enhanced Care
Examples: Incontinence support or toileting, three to five baths per week, more complicated medications including special medications such as eye drops or more than 6 medications daily, full assistance with personal grooming and hygiene, specialty diets, cueing assistance for memory loss and cognitive impairment. Enhanced care has an additional cost per day.
Advanced Care
Examples: Full incontinence support, diabetic assistance, daily bathing, pericare controlled medications or eight or more medications daily. Advanced care has an additional cost per day
