Sylvan Crossings homes are licensed in Wisconsin and located throughout the state. We offer assisted living, short-term stays after hospitalization or prior to deciding to move here, respite care, and hospice care. Some homes have an on-site rehabilitation clinic. Wellness is one of our focus points and keeping residents as independent as possible is one of our goals.

Assisted living is a practical alternative for seniors who do not require skilled nursing assistance but need some assistance, or for those who don't feel safe living by themselves. Residents receive whatever type of assistance they need, such as bathing, grooming, dressing and medication reminders or assistance. They share in home cooked meals, enriching social activities and creative outlets, yet they maintain the ability to make their own choices as they did in their own homes.

All Sylvan Crossings homes are overseen by TouchStone Senior Services, LLC, a professional management company for assisted living homes.
