Sylvan Crossings specializes in the care of older adults and is licensed by the State of Wisconsin to provide this specialized service. We are dedicated to providing the highest quality, most sensitive individualized services to the people who reside with us. We promote and encourage autonomy, decision-making, independence and provide opportunities for social activities. We care about our residents, value their accomplishments and strive to enhance their lives.
Our personal care and supportive services meet the needs of older adults who have some limitation due to their age. The homes are accessible and accommodate walkers and wheelchairs.
We screen and select our residents to carefully match services and resident needs. The admissions process is essential to our philosophy of care and delivery services. The process is key to the supportive environment we create for our residents, their families and our staff.
Our staff monitor the health and well-being of residents each day. Their observations and concerns are regularly shared with the resident, consulting nurse, and highlight care by family and our resident supportive network.
